Question Types

There are numerous Question Types that could be created for the question set. Here you will find more details on how each one works, looks like and how to create.

Question Types

There are numerous Question Types that can be selected from when creating the question set.

Here is a quick list and explanation of the different types.
  1. Long Text
    • input sentences or paragraphs
  2. Short Text
    • input text or numbers
  3. Single Choice
    • Only one answer can be selected out of the multiple options provided. (Similar to radio button option found in other forms.)
    • Other option can be added, this means the user can can type in their own answer
    • Logic can be applied
      • this means based on the answer provided the user will be redirected to questions found on another page.
  4. Multiple Choice
    • For answers that allow more than one answer to be selected. (Similar to check box option found in other forms.)
    • Other option can be added, this means the user can can type in their own answer
  5. Grid - Single Choice
    • Only one answer can be selected out of the multiple options provided.
    • Example:
      • Please rate the following in the restaurant - Food Quality, Food temperature, Food presentation
      • They could choose one of the following - Poor, Below Average, Average, Above Average, Excellent
  6. Grid - Multiple Choice
    • Same answer choices are used for multiple questions and they can select more than one answer
    • Example:
      • Please select all answers that apply
        • What areas do you visit the most?
        • Where can we improve?
      • They could choose one or all the choices provided - Dining, Fitness, Golf, Spa
  7. Grid - Text
    • Input text or number for one or multiple questions
  8. Grid - Number
    • Only numbers can be entered as the answer for one or multiple questions
    • Recipients can also click or up/down arrow to select number
  9. Grid - Date
    • Only a date can be entered as the answer mm/dd/yyyy for one or multiple questions
    • Option to change order of questions within the grid
    • Recipients can also click on calendar icon to select date 
  10. Grid Time
    • Only time can be entered as the answer 00:00 AM/PM
    • Recipients can also click or up/down arrow to select time
  11. Sliding Scale
    • Click and slide scale to select number 
    • Min and max numbers can be set for the scale
  12. Star Rating
    • Stars will turn yellow as user hovers over, user clicks to select number of stars
    • Max number of stars 10

Note: Grid question types are usually used when similar questions of the same question type are used a few times, instead of appearing as separate section they can be presented in a table format.


 
The following are some examples on how to create the question and what it would look like in preview mode.

Create and Preview

Long Text and Short text questions or questions with open ended answers where the recipient can type in an answer that contains letters or numbers.
The difference is long text answers will more likely be longer and in sentences and paragraphs where as short text answers will be a few words or numbers.

The difference in the preview mode is the Long Text answer box will be bigger than the short text answer box.
Single Choice questions are designed to ask a question and only one answer can be selected.
There is the option of creating an Other option where the recipient types in their own answer.

Preview Single Choice
Single Choice questions are designed to ask a question and only one answer can be selected. However, logic can be added to the single choice questions when using MemberInsight.

Once the answer is selected for the single choice question and they click on Next at the bottom of the page it can redirect them to another page in the question set based on their answer.

Only two extra things have to be added to a Single Choice question type to set this up.
  1. Go to page based on answer box needs to be checked
  2. Identify which page to direct them based on their answer.
Note: the additional pages should be created first otherwise when you click continue to next page the only option will be go to page 1 and Submit form. To create a new page, click on Add page in the bottom right corner.

Go to page based on answer - check box to enable logic
  • Continue to next page appears beside answers
  • Select page # or Submit Form to redirect members
  • If page has not been created 
    • Click Add page in the bottom right
Example: page 3 and page 4 have not been set up to redirect for the Fitness and Golf questions. Click Add pag.

Click to add option: Click in box to add another answer

Add 'Other': Click Add 'Other'
  • The last answer in this question will say Other
  • Recipient can type in their answer
  • Logic cannot be built in to Other answer
After page 1: If the recipient selected Other as the answer
  • Decide which page to go to next in the survey (Continute to next page, a specific page number or Submit Form)
    • Continute to next page
    • A specific page number
    • Submit Form
  • Remember to set this up for each page
  • By default the last page in the survey will submit the form. That is why there is no after page 2 option on the right.


 
Preview Single Choice with Logic
Note: notice the next button after Other was selected
This is because After page 1 was set to Continue to next page. When the recipient clicks on Next it will now start asking them the Main Dining questions on page 2. If the intention was to submit the form after all questions are answers on page 1 remember to change the After page 1 option to Submit form.
 
Multiple Choice questions are designed to ask a question and one or all the answers can be selected.
There is the option of creating an Other option where the recipient types in their own answer.

Grid Single Choice questions are designed to ask a few questions where one answer can be selected and the same answers can be selected from for all the questions. The information will now be displayed in a table format where the questions are on the left and the answers will have their own column.

Preview Grid Single Choice
Grid Multiple Choice questions are designed to ask a few questions where one or all the answers can be selected and the same answers are used for all the questions. The information will now be displayed in a table format where the questions are on the left and the answers will have their own column.

Require an answer to this question:

  • Always uncheck this box.

If this box is checked that means every single box has to be checked in order to move on to the next page or submit the form.


Preview Grid Multiple Choice
There are a few other Grid options and they are created with the same format.
  • Grid Text
  • Grid Number
  • Grid Date
  • Grid Time
They are designed to ask a few questions where one answer can be selected and the same answer format can be typed in for all the questions The information will now be displayed in a table format where the questions are on the left and the answer boxes will be on the left.

Click to add column: Click in box to type in instructions on how the question should be answered in column 1.
  • Usually only one column would be used but if a second column of answers are required add information to column 2 or 3 respectively

Preview Grid Number
Sliding Scale questions are designed to ask a number rating question and the answer can be selected by moving the slider on the screen.

  • Enter minumum and maximum number range for answer

  • Check - question is mandatory and user has to answer
  • Uncheck - question is optional, user does not have to answer

Star Rating questions are designed to ask a rating question and the answer can be selected by the number of stars that turn yellow.

  • Maximum number of stars is 10
