The Questionnaire Builder allows the creation of additional custom questions that can be added to surveys. It offers an easy drag-and-drop interface to design and organize questions, apply logic, and customize settings. This tool helps enhance surveys by tailoring question sets to specific needs without requiring technical skills.

The section below provides a high-level overview of how to create and manage a questionnaire in the system. To view detailed instructions see here (Recommended). 

  1. From the main menu, select Comms > Questionnaires.
  2. Select Create Questionnaire.
  3. Or select title of an existing questionnaire to edit.
  4. Use the Questionnaire Builder to create or edit a questionnaire.
  1. Within the Questionnaire Builder, select the Designer tab.
  2. In the top-right corner, select the Show Panel (arrow left) icon to open the Property Grid.
  3. Under the General category:
    • Survey Title: Enter the title that will be visible to recipients.
    • Survey Description (optional): Add context or instructions for recipients.
  1. Select the Designer tab to add and organize questions.
  2. Use the Toolbox on the left to drag and drop an element such as questions, pages, and panels to the form.
  3. Select any element to view its settings in the Property Grid on the right.
  4. If the Property Grid isn’t visible, select the Full Screen button, then the Show Panel icon.
  5. Customize each question by editing:
    • Titles and descriptions
    • Answer types (e.g., multiple choice, dropdown)
    • Default values and visibility options
Use the Logic tab or Conditions in the Property Grid to:
  • Show or hide questions based on responses
  • Add validation such as required fields or input ranges
  1. Select the main survey item to open its Property Grid.
  2. Under Navigation, configure:
    • Page navigation settings
    • Progress bar visibility
    • Time limits (if required)
  1. Select the Preview tab to test the questionnaire.
  2. Answer the questions and select Complete to review test results.
  3. Choose Preview Survey Again to restart.
  4. Preview results are temporary and not saved
  1. Select the Collapse button in the top-right corner to exit full screen.
  2. At the bottom-right, choose a status:
    • Draft: Keeps the questionnaire in edit mode and unavailable for surveys.
    • Ready: Finalizes the questionnaire for use in a survey. Once set to Ready, only the name can be changed.
  3. Choose one of the following options:
    • Save: Save progress and remain on the same page.
    • Save & Exit: Save and return to the questionnaire list.
